Get to know your provider before signing up:

We offer all new patients a Consultation Before Community Membership (CBCM).

This visit is designed to let you experience the practice before you commit to the Community Membership. You will meet face-to-face with a medical provider, review your health issues and treatment options, and determine together the level of care (care track) you need or desire.

This visit is submitted to your insurance (copay, co-insurance, deductible apply). Your provider will review your health history and consult on treatment options.
